Initial Comments:
An off-site follow up survey conducted May 24, 2023 found that Brightstar Care of Happy Valley, had corrected the deficiencies cited under the requirements of 28 Pa. Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a state re-licensure survey conducted on April 5, 2023
